At OGB, we work to help our members improve and preserve their quality of life. Below are resources available to OGB plan members.
In Health: Blue Health Services
The Disease Management program administered by Blue Cross Blue Shield of Louisiana is available to PPO, HMO, CDHP members and non-Medicare primary retirees at no cost. The program helps members with diabetes, coronary artery disease (CAD), chronic heart failure (CHF), asthma, and COPD improve and manage their health through health coaching with BCBS nurses and with educational materials.
By participating in the disease management program, members are 65 percent less likely to be admitted to the hospital. Participation in the disease management program can save money on certain prescriptions. Prior to August 1, 2014, PPO, HMO, and CDHP members receive certain no cost generic prescriptions and $15 name-brand prescriptions. Beginning August 1, 2014, PPO and HMO members will continue to receive certain no cost generic prescriptions and will receive certain name-brand prescriptions for $20 (31 day supply) / $40 (62 day supply) / $50 (93 day supply). The current disease management prescription incentive for the CDHP plan will remain intact. To participate in the program, you must be diagnosed with one of the above conditions and agree to talk to a health coach once every 90 days. If you are interested in enrolling in the In Health program, call BCBS at 800-363-9159 from 8:00 AM until 5:00 PM Monday through Friday (except holidays) or visit their website.
Diabetic Management Program
OGB’s diabetic management program (formerly known as diabetic sense) is available to HMO and PPO members who have diabetes. Members DO NOT have to be enrolled in the Disease Management Program to take advantage of the benefits offered with the diabetic management program. There are a number of advantages to enrolling in both programs, but it is not required.
Under the diabetic management program, members are able to get their diabetic supplies at a $0 co-pay through selected mail order suppliers or at their local retail network pharmacy.

Tobacco Cessation Pilot Program for State Employees
State employees who are ready to quit smoking or using tobacco are encouraged to enroll in a tobacco cessation program through the Louisiana Tobacco Quitline (1-800-QUIT-NOW). The quitline’s certified quit coaches work with each caller to develop a free personalized quit plan. Program participants receive up to five free telephone coaching sessions and other resources.

Smoking Cessation Trust
If you are a Louisiana resident who developed a smoking habit before September 1, 1988, and would like to stop, you may be eligible to receive cessation medications, individual or group cessation counseling, telephone quit-line support and/or intensive cessation support services at no cost. Heads Up Weight Loss Study
OGB members enrolled in the PPO or HMO plan may be eligible to participate in an OGB weight loss program now being conducted by Pennington Biomedical Research Center in Baton Rouge. (Additional clinic sites are open in Lafayette and Lake Charles and clinics are in the works for Alexandria, Covington, Monroe, New Orleans and Shreveport.) The program is for those who are severely overweight and suffering weight-related health problems. It includes a bariatric surgery component as well as intense medical management treatment.

Live Better Louisiana
Live Better Louisiana is an OGB program that will give Blue Cross plan members resources to help you better monitor your health, understand your risk factors and make educated choices that keep you healthier. It's sponsored by Blue Cross and Blue Shield of Louisiana at no extra charge to you.

Vantage Health Plan Disease Management Programs
Vantage Health Plan’s Disease Management Programs (DMPs) are educational programs for members with chronic (long-term) condition(s). The purpose of the DMPs is to help members better self-manage their condition(s).
Once enrolled in one of the DMPs, a clinical pharmacist will contact the member to talk about their chronic condition(s). The pharmacist will also send educational and health-reminder mailings, perform a complete medication review, and offer daily self-care tips to help better manage their condition(s) and set health care goals.
