When a newborn is added as a dependent, OGB must receive a copy of the birth certificate within 6 months of the child’s birth date.
The birth letter will suffice for the first 6 months only if OGB receives a copy within 30 days of birth.

If you see something on an Explanation of Benefits (EOB) that doesn't look right or if it lists a service that you don't think was performed,
contact your doctor or health care provider to question it.
